*To qualify, you must: Sign up at www.eayso.org, bring two signed copies of the registration form on May 17th to the Menifee Valley Middle School , with a copy of the player's Birth Certificate, pay in full by cash or check to AYSO, and complete the evaluation.  
Evaluations will only be held on May 17th.
